I am a community rheumatologist and have prescribed avacopan since its initial approval in 2021. While I fully recognize, and has personally witnessed the potential for liver toxicity, I do hope the FDA continues to allow for the use of avacopan. I have seen the incredible benefit of avacopan. I have taken care of patients who suffered terrible ongoing vasculitis despite standard of care (rituximab, steroids) , who then only went into remission after adding avacopan. I have seen patients with ANCA vasculitis who were dialysis dependent despite rituximab and steroids, who experienced such remarkable renal recovery following avacopan that they were able to get off of dialysis. The liver toxicity is important, but this potential adverse effect is something that can be monitored for more closely.
